Sample Language for Your Will or Trust
The following language may help you and your attorney when drawing up a bequest that meets your needs.
“I give to the Muscular Dystrophy Association, Inc., Tax ID: #13-1665552, a nonprofit corporation currently located at 1016 W Jackson Blvd #1073,
Chicago, Illinois 60607, or its successor thereto, ______________* [written amount or percentage of the estate or description of property] for its unrestricted use and purpose.”
